New Patients: Once your appointment request is approved, you will receive a confirmation email containing our building entry codes. Please download, complete, and print or email the new patient intake form to expedite the process. It is also available at the clinic for you to fill out.

ONLINE BOOKING POLICY

If you don’t see your preferred day or time available online, or if you are looking to get in sooner, please call or text us directly at 206-708-2586 so we can look for an opening or add you to our priority list. Cancellations or rescheduling require a minimum of 24 hours' notice.
