To expedite your first visit, please complete your secure online intake paperwork prior to your arrival. Your information is fully encrypted and HIPAA-compliant.

3. Notice of Privacy Practices Acknowledgment (Required)

Note: If you are unable to complete these online in advance, paper forms are also available to complete at the clinic when you arrive.

If you don’t see your preferred day or time available online, or if you are looking to get in sooner, please call or text us directly at 206-708-2586 so we can look for an opening or add you to our priority list. Cancellations or rescheduling require a minimum of 24 hours' notice.
